An 11-year-old girl suffered a traumatic amputation of her left index finger at a Northern Illinois facility when her finger caught on an exposed, non-flush screw-head while climbing a ladder-like obstacle.

Settlement paid by a confidential defendant. Attempts to reattach the finger were unsuccessful, and she now wears a prosthetic finger, which will require lifelong replacements.
