Case against North Canyon Care Center (Avalon Care Center - Bountiful 350 South, LLC dba North Canyon Care Center). Mr. Sprague, age 57, suffered from end-stage multiple sclerosis and was at high risk for pressure sores. Staff failed to turn him every two hours as instructed, leading to new stage IV pressure ulcer and deterioration of existing ones. He died 20 months later.
