An 82 year-old man with progressive dementia who was admitted to a long term acute care facility (LTAC) for ventilator management died when his tracheostomy tube became dislodged. Although the tube was quickly replaced, the patient was unable to be resuscitated. He was survived by a spouse and three adult children.
