Rollover, T-bone and head-on hotspots in San Antonio
A crash in San Antonio is rarely random about where it happens. The same freeway ramps, the same wide arterials, and the same intersections turn up over and over in the crash records, and the type of wreck usually matches the road that produced it.
In 2024, Bexar County recorded 48,522 crashes. Of those, 205 were fatal, 215 people died, and 773 crashes left someone with a serious injury.1 Inside the San Antonio city limits, the count was 162 fatal crashes, 170 deaths, and 596 serious-injury crashes.2 For scale, Texas as a whole lost 4,150 people on its roads that year, roughly one death every two hours.3
Three crash types do most of the killing and maiming here: side-impact (T-bone) collisions at intersections, head-on wrecks (many of them wrong-way), and rollovers. Each has its own geography.
T-bone crashes and the arterials that feed them
A T-bone, or angle crash, happens when the front of one vehicle hits the side of another, usually because a driver ran a light, misjudged a gap, or turned across oncoming traffic. The side of a car has far less structure than the front, so whoever sits next to the impact takes the worst of it. Statewide, 1,050 people were killed in 2024 in crashes at or related to an intersection.3
San Antonio's own data points straight at the roads where this keeps happening. The city's Vision Zero team built a High Injury Network by weighting five years of TxDOT crash records (2019 through 2023) and pulling out the street segments with the heaviest concentrations of fatal and serious-injury crashes.4 The corridors that scored highest are a familiar list to anyone who drives the West and South Sides: SE Military Drive, Bandera Road, W.W. White Road, Culebra Road, Zarzamora Street, and Rigsby Avenue, plus Fredericksburg Road on the near Northwest Side.4
These are wide, fast, signalized arterials with lots of driveways and turning traffic, which is the recipe for angle crashes. On one high-scoring corridor the city examined, 15 fatal or severe crashes were tied to turns: 60 percent involved left turns and half involved right turns, with speeding a factor in 30 percent.4 The fixes the city is testing (center medians, narrower lanes, left-turn control, and roundabouts) all aim at those turning conflicts.
Head-on and wrong-way wrecks on 281 and the freeways
Head-on crashes are the rarest of the three types and the deadliest per wreck, because two vehicles combine their speed at the point of impact. Texas counted 617 head-on deaths in 2024.3
In San Antonio, a large share of the worst head-on wrecks come from wrong-way drivers on the freeways. TxDOT's San Antonio District built an entire wrong-way driver program after Police Officer Stephanie Brown was killed on March 15, 2011, struck by a driver who had entered IH-35 going the wrong direction only seconds earlier.5 When the agency mapped where wrong-way events cluster, US-281 north of downtown stood out as the worst. The ramp area at US-281 and Airport Boulevard logs more reported wrong-way events than any other single location in the city, and the corridor alone accounts for more than 20 percent of all wrong-way reports.6 The stretch of IH-35 north of downtown, between IH-10 and IH-37, had the highest number of actual wrong-way crashes, and IH-10 west toward Huebner and US-90 out to Loop 1604 also showed heavy activity.6
The pattern behind these wrecks is steady. More than 80 percent of San Antonio's wrong-way reports come in between 10 p.m. and 6 a.m., and 45 percent land in the narrow window between 2 a.m. and 4 a.m., right around closing time.7 TxDOT ties the problem directly to drunk driving,7 which matches the statewide picture: 1,053 people were killed in 2024 in Texas crashes involving a drinking driver, and more of those crashes happened between 2 and 3 a.m. than any other hour.3 On the 15-mile US-281 corridor from IH-35 up to Stone Oak, TxDOT installed radar detectors and flashing LED wrong-way signs at the exit ramps, plus mainline message signs that light up and alert TransGuide operators and SAPD dispatch the moment a wrong-way vehicle is detected.8
Rollovers on the fast edges of town
Rollovers are a highway and rural-road problem, and they hit Texas harder than most states because so many people here drive pickups and SUVs. Those taller vehicles carry a higher center of gravity and tip more easily once they leave the pavement. The Insurance Institute for Highway Safety found that rollovers accounted for 28 percent of passenger-vehicle occupant deaths in 2024, and the share was far higher in single-vehicle crashes (47 percent) than in multi-vehicle ones (13 percent).9 Broken out by vehicle, rollovers were involved in 38 percent of pickup occupant deaths and 33 percent of SUV occupant deaths, against 20 percent for cars.9
That lines up with how people die on Texas roads generally. Single-vehicle, run-off-the-road crashes killed 1,353 people in the state in 2024, about a third of all traffic deaths, and rural crashes accounted for just over half of the total.3 In and around San Antonio, the rollover risk climbs on the outer ring: the fast, sweeping ramps of Loop 1604, the rural FM roads on the developing North and far West Sides, and the highway curves where a single overcorrection at speed can put a truck on its roof.
Who handles this, and what is changing
San Antonio adopted Vision Zero in 2015 and set a goal of ending traffic deaths and serious injuries by 2040.4 The work is split across several agencies. The City of San Antonio Transportation Department owns the local arterials and the High Injury Network redesigns, TxDOT's San Antonio District runs the freeways and the wrong-way program, and the San Antonio Police Department and Bexar County Sheriff handle enforcement and crash reporting. The regional planning body, the Alamo Area Metropolitan Planning Organization, ties the funding together.
None of that undoes a wreck that already happened. If you have been hit, get the CR-3 crash report, see a doctor even if you feel fine, and write down what you remember about the other vehicle and the signal while it is fresh.
